One record per distinct qualifying patient (typically, attributed patients) in the calendar month with the highest count of distinct Patient ID values from among the last X months prior to the most recent ELT. The X-month "evaluation period" is meant to reflect a reasonable lookback period for analytics, and also to make the counts robust to data lag that might result in undercounting in more recent periods.
