Measures with geographic fields that have been identified in Case Review can be viewed in the map tab of Measure Explorer, which can plot patient, provider, and secondary provider locations.
Setup
Some geographic data underlying the map is pulled from the Zip Code Reference table, so this object must be run before trying to view maps.
Geographic Fields
To activate the patient map screen, a geographic field of the patient, provider, or both must be identified on the measure in Measure Workshop. For provider pushpins to show up, both the provider name and either the provider zip or latitude/longitude must be identified. For secondary providers to show up, both the secondary provider name and the secondary provider zip or latitude/longitude fields must be identified. If available, latitude and longitude are preferable to the provider zip because they provide better precision. It is assumed that patient latitude and longitude are never known, and zip will suffice.
Choropleths vs Patient Maps
Patients are grouped by their zip codes on the map. With the patient map chart type, the user will see these groups as volume bubbles, where larger bubbles represent a larger case volume. Item coloration reflects the measure value, as described in the map legend. With the choropleth chart type, the user will see the outlines of the zip codes. Coloration reflects the measure value with choropleths as with patient maps, and choropleths do not have the capability to show case volume.
Provider Details
Each provider is identified by a pushpin. The color of the pushpin changes depending on the relative case volume attributable to the provider, with darker colors indicating greater volume. A user can click on the pushpin to add a filter to only see the patients attributable to that provider on the measure.
Secondary Provider Details
Secondary providers may be identified on the measure. For example, if the primary provider represents a hospital on a discharge measure, the secondary provider might represent SNFs. Users can select to view the secondary provider details instead of the patient details via an option in the hamburger menu.
Navigation
As users hover their mouse over a provider, secondary provider, or patient on the map, the aggregated details for that patient zip code or provider are shown in a detail box. Users can click on the item to freeze the detail box. Clicking on the same item again will unfreeze the detail box. From the detail box, users can drill down into Case Review with the pertinent filter applied.
