[SO/URSA-HCC] Patient Timelines of Initial, Midyear, and Final Run RAF Results from MMR

Object Description

One record per patient-month for which the patient received an MMR record covering that month. Calculates the initial, midyear, and final run HCC/RAF scores for each covered month.
