Measure Description

Among hospital inpatient admissions, the percent with one or more subsequent admission in the 12 months following the Index Admission discharge date. If a patient has multiple subsequent admissions, the contribution to the numerator will be remain 1 though information on all subsequent admissions will be displayed in case review.

Denominator Description

Hospital inpatient admissions for plan members with medical claim coverage, excluding admissions with a discharge status indicating the patient expired, was transferred to inpatient status at another hospital, or had a planned readmission.

Numerator Description

If the patient has one or more subsequent IP all-cause admissions in the twelve months following the day of discharge then 1, else 0.

    • Patient Age Category Tier 2 -- Non-overlapping ranges of the patient age in years representing finer grain categories than the Patient Age Category Tier 1.
