Measure Description

The percentage of members who were screened, using prespecified instruments, at least once during the measurement period for unmet transportation needs and received a corresponding intervention if they screened positive.

Denominator Description

Patients of all ages with a positive transportation insecurity screen finding, using prespecified instruments, at least once during the 11-month period ending 1 month prior to the snapshot date.

Numerator Description

Qualifying denominator patients who received a transportation insecurity intervention on or up to 30 days after the date of the first positive housing screen.
