[MI/URSA-QUALITY] HEDIS HCC Combinations Reference

Object Description

One record per NCQA HCC pairs whose combination results in an additional NCQA HCC representing the marginal contribution to the patient's total risk of that interaction. The reference table includes a slot for a third HCC (i.e., for a 3-HCC combination), following the structure of the source data file, but this third HCC slot is not currently used.
