Is Advanced Illness

Two or more instances of advanced illness on distinct dates in the last 24 months or one or more instance of dementia medications in last 24 months. Typically combined with age and/or frailty for denominator exclusion.

Is Frailty

Two or more instances of frailty on distinct dates in last 12 months. Typically combined with age and/or advanced illness for denominator exclusion.

NCQA Ethnicity Code

The code used to identify ethnicity in official NCQA measure submissions; e.g., 1 = Hispanic or Latino, 2 = Not Hispanic or Latino, 3 = Unknown Ethnicity.

NCQA Ethnicity Data Source Code

The code used to identify the source of ethnicity data used in official NCQA measure submissions; e.g., 91 = CMS Databases, 92 = State Databases, 93 = Surname Analysis, etc.

NCQA Ethnicity Data Source Direct vs. Indirect Type Code

The code used to identify the type -- in terms of direct vs. indirect sources -- of the data source used to obtain patient ethnicity data used in official NCQA measure submissions; e.g., 1 = Direct, 2 = Indirect, 3 = Unknown.

NCQA Gender Code

The code used to identify gender in official NCQA measure submissions; e.g., M = male; F = female.

NCQA Measure ID

The value used by NCQA to identify the measure and, in cases where a measure includes multiple rates, the rate; e.g., CBP = Controlling Blood Pressure measure, CISDTP = Childhood Immunization Status measure DTaP rate, CISOPV = Childhood Immunization Status measure IPV rate, etc. Not to be confused with the NCQA Measure Code, which identifies only the measure, not the rate; in particular, it is the NCQA Measure ID, not the NCQA Measure Code, that is included in official HEDIS measure results submissions.

NCQA Race Code

The code used to identify race in official NCQA measure submissions; e.g., 1 = White, 2 = Black or African American, 3 = American Indian and Alaska Native, 4 = Asian, etc.

NCQA Race Data Source Code

The code used to identify the source of race data used in official NCQA measure submissions; e.g., 21 = CMS Databases, 22 = State Databases, 23 = Surname Analysis, etc.

NCQA Race Data Source Direct vs. Indirect Type Code

The code used to identify the type -- in terms of direct vs. indirect sources -- of the data source used to obtain patient race data used in official NCQA measure submissions; e.g., 1 = Direct, 2 = Indirect, 3 = Unknown.

Patient Ethnicity CDC Race and Ethnicity Code

The CDC Race and Ethnicity Code identified as the patient's ethnicity.
