  • Patient ID -- The internal database identifier (used, e.g., for joins and primary keys) for the patient. This value is typically mastered, i.e., all records for the same patient, regardless of the source data system from which that record originated, should have the same Patient ID value. (Note that while the mastered Patient ID value might resemble a local identifier used in one of the upstream data sources, this does not indicate any special priority of that source system in determining the characteristics of the patient.)   • Source ID -- The identifier for the original source data system from which the current record originated.

  • Patient Race 2 CDC Race and Ethnicity Code -- The CDC Race and Ethnicity Code identified as the patient's second race. (See also [URSA-QUALITY] CDC Race and Ethnicity Code)
  • Patient Race 3 CDC Race and Ethnicity Code -- The CDC Race and Ethnicity Code identified as the patient's third race. (See also [URSA-QUALITY] CDC Race and Ethnicity Code)
  • Patient Ethnicity CDC Race and Ethnicity Code -- The CDC Race and Ethnicity Code identified as the patient's ethnicity. (See also [URSA-QUALITY] CDC Race and Ethnicity Code)
  • NCQA Race Data Source Code -- The code used to identify the source of race data used in official NCQA measure submissions; e.g., 21 = CMS Databases, 22 = State Databases, 23 = Surname Analysis, etc.
  • NCQA Ethnicity Data Source Code -- The code used to identify the source of ethnicity data used in official NCQA measure submissions; e.g., 91 = CMS Databases, 92 = State Databases, 93 = Surname Analysis, etc.
  • Is Patient Assigned Female at Birth
  • Is Patient Assigned Male at Birth
  • NCQA Gender Code -- The code used to identify gender in official NCQA measure submissions; e.g., M = male; F = female.
