URSA-HCC-004: Visits Capturing All Missing Historical HCCs

Measure Description

Among clinician office visits for patients with one or more historical HCCs undocumented in the current calendar year, the percentage with all historical HCCs documented following the visit.

Denominator Description

Clinician office visits for patients with one or more historical HCCs that were undocumented in the calendar year-to-date, as of the day prior to the visit.

Numerator Description

One for each qualifying denominator visit in which the patient had no missing diagnoses as of the date of the visit (reflecting the patient status after the visit).
