Measure Description

Among patients aged 18 to 85 with hypertension, the percentage with controlled blood pressure on their most recent reading.

Denominator Description

One per patient aged 18 to 85 with two or more qualifying visits with a diagnosis of hypertension in the 18-month period ending 6 months prior to the snapshot date.

Numerator Description

One per qualifying denominator patient with documentation of blood pressure < 140/90 in the past 12 months and after the second denominator visit.
